Job summary

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling up to 50% to meet participants.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and goals.
• Develop, implement, and update ISP so participants receive the right services.
• Provide participants with choice and advocacy for informed provider decisions.
• Coordinate with medical professionals, behavioral specialists, family, and community resources.
• Monitor services to ensure safety, satisfaction, and progress toward independence.
• Support adults with disabilities and aging individuals in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
