Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Willingness to travel up to 50% for home and community visits.
• Valid driver's license and auto insurance.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify needs, goals, and preferences.
• Develop, implement, and update ISP plans to ensure appropriate services.
• Provide participants with choice and advocacy in providers and care options.
• Coordinate with medical professionals, behavioral specialists, families, and community resources.
• Monitor and evaluate services to ensure safety, satisfaction, and progress.
• Assist individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Engage in ongoing training and professional development.
