Job summary

Care Lync is seeking a compassionate Support Coordinator to help adults and seniors access housing, medical care, and community services. You will develop Individual Support Plans, advocate for participants, and coordinate with providers to ensure seamless care in the community.

The role emphasizes person-centered assessments, strong communication, independence, and ongoing professional growth through training and mentorship. Travel to participants’ homes is required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Ability to travel up to 50% to meet participants in homes and communities.
• Valid driver’s license and reliable transportation.
• Excellent organizational, communication, and advocacy skills.

Responsibilities

• Conduct person-centered assessments to identify participants’ needs, preferences, and goals.
• Develop, implement, and update ISP plans to ensure appropriate services.
• Provide participants with choice and advocacy in selecting providers and care options.
• Coordinate with medical and behavioral professionals, families, and community resources to deliver high-quality care.
• Monitor services to ensure safety, satisfaction, and progress toward independence.
• Maintain documentation and ensure confidentiality and compliance with standards.
• Participate in ongoing training and professional development.
