Job summary

Care Lync is seeking a Support Coordinator to help adults with disabilities and seniors access housing, medical care, and community resources. You will conduct person-centered assessments and develop Individual Support Plans to secure appropriate services.

You'll collaborate with medical professionals, families, and community partners, travel to homes up to 50% of the time, and maintain compliant records while growing your career within a supportive team.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Ability to travel up to 50% to meet with participants.
• Strong organizational and advocacy skills.
• Comfort working independently.

Responsibilities

• Conduct person-centered assessments to identify participant needs and goals.
• Develop and update Individual Support Plans to ensure services meet needs.
• Provide participants with choice and advocacy in care options.
• Coordinate with medical professionals, behavioral specialists, families, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress.
• Support both individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Participate in ongoing training and professional development.
