Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator to develop individualized care plans and coordinate services for adults with intellectual, developmental, and physical disabilities. You will broker and monitor services, ensuring cost-effective, professional delivery and client empowerment.

Responsibilities include creating person-centered plans, coordinating with care teams, and ensuring confidentiality.

Qualifications

BS/BA degree in related course work
Minimum of 3 years related experience providing case management for individuals with disabilities
Experience with Microsoft Office and state-maintained databases
Maintain CPR/First Aid certification and updated state related clearances
Valid Driver's License including personal vehicle insurance coverage and current vehicle registration

Responsibilities

Complete assessments to identify participant needs and desires for service delivery
Develop Person-Centered Service Plans and modify annually or with significant changes
Ensure participant choice of providers by providing information for informed decisions
Coordinate with care team to ensure seamless delivery of physical, behavioral and support services
Maintain confidentiality of sensitive information and participate in required trainings
Travel to participants as needed (up to 50%)
