Job summary

SCO Family of Services in New York is seeking a direct support supervisor to oversee individuals according to individualized treatment plans. In 1:1 cases you will provide continuous supervision and complete transfer forms when care changes hands.

Responsibilities include participating in program planning meetings, signing off on annual and semi-annual plans, maintaining possessions, transporting clients safely, and ensuring compliance with OPWDD regulations.

Qualifications

• Minimum high school diploma or GED.
• Clean NYS driver's license preferred.
• Experience working with people with developmental disabilities/traumatic brain injuries preferred.
• Acceptable background clearance per State Central Register and health regulations for exam and Mantoux.

Responsibilities

• Provide close supervision for individuals per the individual treatment plan; 1:1 supervision as noted; complete transfer forms when transferring care.
• Participate in program planning meetings; review and sign off on Annual and Semi-Annual Treatment Plans.
• Maintain individuals' possessions neatly; communicate needs to management; complete documentation per procedures.
• Transport individuals safely; comply with NYS laws and agency procedures; maintain valid NYS Driver's License; complete vehicle logs and related documentation.
• Maintain the physical plant and agency property in a clean and safe manner; report issues to management.
• Provide other duties as needed to ensure safety, health, and comfort of individuals.
• Comply with OPWDD regulations and agency procedures.
• Obtain and maintain SCIP-R certification; attend mandatory training.
• Obtain and maintain AMAP certification and submit copies to programs where meds are administered.
• Attend all staff meetings and trainings; demonstrate competency in required training areas.

Education
