Job summary

SCO Family of Services seeks a dedicated staff member to provide close supervision to individuals per individualized treatment plans in a New York setting. The role requires participation in program planning, documentation, and adherence to state regulations.

A clean NYS driver's license and experience with developmental disabilities are preferred. The position includes transporting individuals safely, maintaining vehicle and program documentation, and supporting team training and staff

Qualifications

• Minimum high school diploma or GED.
• Clean and valid NYS driver’s license required.
• Experience with developmental disabilities/traumatic brain injury preferred.
• Ability to complete SCIP and AMAP training.

Responsibilities

• Provide close supervision per the individual treatment plan and 1:1 supervision as needed.
• Participate in program planning meetings and sign off on Annual/Semi-Annual Treatment Plans.
• Maintain individuals' possessions and communicate needs to management; document per procedures.
• Transport individuals safely; maintain vehicle logs; ensure a comfortable and safe environment.
• Maintain the physical plant and agency property; report issues to management.
• Attend staff meetings, trainings, and mandatory certification courses (SCIP/AMAP).
• Comply with OPWDD regulations and agency procedures.
• Travel up to 50% of the time.
