Job summary

SCO Family of Services in New York, NY is seeking a part-time Direct Service Worker to provide close supervision and support under individualized treatment plans at our Ozone Park location. The role requires on-site work and involves implementing plan directives with individuals daily.

You will participate in program planning meetings, maintain orderly personal belongings, document care activities, and ensure safe transportation following NYS laws.

Qualifications

• High school diploma or GED required.
• ASL preferred.
• Experience working with people with developmental disabilities or traumatic brain injury preferred.
• Ability to complete SCIP and AMAP training.

Responsibilities

• Provides close supervision for individuals per the treatment plan and continues 1:1 supervision as specified.
• Participates in program planning meetings and signs off on annual/semi-annual plans.
• Keeps individuals' possessions neat; communicates needs to management; documents per procedures.
• Transports individuals safely; follows NYS laws; maintains a valid NYS Driver's License; records vehicle logs.
• Maintains the physical plant and agency property; reports issues to management.
• Executes other duties to ensure clients are safe, healthy and comfortable.
• Complies with OPWDD regulations and agency procedures.
• Obtains and maintains SCIP-R and AMAP certifications; attends mandatory training.
• Attends staff meetings and trainings; demonstrates competency in required training areas.
