Job summary

SCO Family of Services is seeking staff to provide close supervision for individuals under a treatment plan in New York. The role involves participating in program planning meetings, maintaining order of possessions, and ensuring documentation is complete and accurate.

You will transport individuals following NYS laws, keep vehicle logs, and report any vehicle issues. The position requires a valid NYS driver’s license and adherence to agency procedures.

Qualifications

• High School diploma or GED required.
• Clean and valid NYS driver’s license required.
• Experience with people with developmental disabilities/traumatic brain injury preferred.
• Ability to complete SCIP and AMAP training.

Responsibilities

• Provides close supervision for individuals in accordance with the individual treatment plan.
• Participates in program planning meetings; reviews and signs off on Annual and Semi-Annual Treatment Plans.
• Maintains individuals' possessions in an orderly manner; communicates needs to management; completes documentation.
• Transports individuals safely; follows NYS laws; maintains vehicle logs; communicates vehicle conditions to management.
• Maintains the physical plant and agency property in a clean and safe manner; reports issues to management.
• Provides other duties as needed to ensure individuals are safe, healthy, and comfortable.
• Complies with OPWDD regulations and agency procedures.
• Attends mandatory training; maintains SCIP-R and AMAP certification; submits copies of certifications to programs.
• Attends all staff meetings and trainings; demonstrates competency in required training areas.
