Job summary

SCO Family of Services is seeking a Direct Support Professional to provide close supervision according to individual treatment plans in a New York setting. You will participate in program planning meetings, assist with treatment plan sign-offs, and ensure accurate documentation and adherence to agency procedures.

Responsibilities include safe transport of individuals, maintaining a clean and orderly environment, and ensuring overall safety and comfort.

Qualifications

• High School diploma or GED is required.
• Clean NYS driver’s license required.
• Experience working with people with developmental disabilities preferred.
• SCIP and AMAP trainings required.

Responsibilities

• Provides close supervision as per individual treatment plan (1:1 if required).
• Participates in program planning meetings and signs Annual/Semi-Annual Treatment Plans.
• Maintains documentation in accordance with program procedures.
• Transports individuals safely and maintains vehicle logs per NYS laws.
• Ensures a safe, healthy, and comfortable environment; communicates needs to management.
• Complies with OPWDD regulations and agency procedures.
• Attends mandatory trainings and staff meetings.
