Job summary

Goodwill Industries of the Summit is seeking a dedicated SUD Case Manager to support clients through a 16-week recovery journey, coordinating services and promoting self-sufficiency. The role emphasizes intake assessments, individualized plans, and ongoing advocacy.

Qualifications include a bachelor’s degree in social work or related field, at least two years in case management, proficiency with Microsoft Office and case management software, and reliable transportation for off-site visits.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or related field.
• Minimum 2 years experience in case management or SUD support.
• Strong organizational skills with the ability to manage multiple cases.
• Excellent written and verbal communication with attention to documentation.
• Knowledge of local recovery resources and support services.
• Proficiency in Microsoft Excel, Word, Outlook and case management software.

Responsibilities

• Conduct intake assessments and develop individualized service plans.
• Provide ongoing case management with goal setting and progress monitoring.
• Maintain participant records using Microsoft Office and case management software.
• Facilitate group and individual meetings to promote recovery and peer support.
• Coordinate referrals to housing, employment, education, and healthcare services.
• Prepare monthly reports to track participant progress and program outcomes.
• Collaborate with staff and partners to ensure holistic participant support.
• Advocate for participants navigating housing, employment, and mental health.
• Ensure compliance with program policies and regulations.
