Sol Recovery Community, Inc. is seeking a dedicated Case Manager & HRSS Manager to support clients on their path to recovery.

Responsibilities include guiding intake, coordinating referrals to residential and detox facilities, and providing on-site case management for our supported living program. The role requires English fluency, strong communication, and computer skills; certification or license (SSW or Case Management) is preferred, with DV Advocacy Training offered if needed.

Competenze

• Must be at least 18 years of age.
• Must hold Social Service Worker license (SSW) or Case Management Certification or willingness to obtain.
• If not certified, DV Advocacy Training will be provided.
• Clean criminal record (no felonies or related misdemeanors).
• TB screening and test if indicated.
• Must pass post-offer and random drug screenings.
• Strong computer skills including Google Workspace, data management, and email.
• Reliable and punctual with excellent communication skills.
• Ability to read, write, and communicate fluently in English.

Mansioni

• Guide clients and families through intake and referrals.
• Ensure completion of intake documentation.
• Develop and maintain relationships with community partners.
• Meet with clients to assess and address needs.
• Provide on-site case management for supported living clients.
• Connect clients to community resources and higher levels of care.
• Coordinate referrals to residential and detox facilities.
• Observe and document urinalysis testing as required.
• Maintain detailed case management notes per SRC policies.
• Collaborate with the clinical treatment team and attend weekly meetings.
• Uphold confidentiality and ethical standards.
• Assist with community outreach and presentations.
• Perform other duties as assigned.
