Job summary

Qualifications

• QMHP-qualified or SUD professional
• Understanding of SUD and co-occurring disorders
• Ability to discuss stages of change and recovery
• Knowledge of reporting and compliance requirements
• Proficiency with Microsoft Office tools
• Strong professional communication and discretion

Responsibilities

• Provide community-based case management for SUD and co-occurring disorders
• Conduct assessments and develop person-centered recovery/treatment plans
• Coordinate transportation and weekly recovery-focused groups
• Complete documentation accurately per CNS policy
• Engage individuals using motivational interviewing and harm-reduction approaches
• Collaborate with healthcare providers and community organizations
