Job summary

Telecare Corporation seeks a Case Manager Substance Use Counselor Certified to serve clients with mental illness focusing on substance use. You will act as the primary support and case manager to foster independent living in the community, coordinating care and services.

Required CADC/CATC/SUDCC or equivalent certification and at least 6 months in addictions settings, with CPR/First Aid/CPI and a valid driver’s license. Full-time with weekday shifts in Los Angeles area.

Qualifications

• High school diploma or GED required; CADC/CATC/SUDCC or CDP per state requirements.
• 6 months’ experience in addictions setting.
• Certifications must be maintained for duration of employment.
• Ability to handle billing codes within 2 months of employment.

Responsibilities

• Provide primary case management and clinical support for individuals with mental illness and substance use issues.
• Assist clients to live as independently as possible in their community.
• Coordinate services and maintain compliance with program requirements.
