Job summary

UHS in Michigan is seeking a Case Manager to provide quality substance abuse case management in line with program policies. You will perform bio-psycho-social assessments, intake tasks, discharge planning, group sessions and contribute to group content.

The role requires collaboration with Admissions, Utilization Review, Nursing and Residential staff to ensure smooth patient transitions and accurate documentation.

Qualifications

• Undergrad degree in social work, counseling, psychology, or mental health with 1 year substance abuse experience.
• Minimum of one year of supervised substance abuse experience.
• Experience performing assessments and group therapy for adults with addiction and mental health disorders.
• Ability to manage high volume client-related paperwork and maintain confidentiality.

Responsibilities

• Assess prospective patients for treatment to determine admission eligibility.
• Complete utilization review pre-certifications based on intake assessment.
• Administers breathalyzer and vital signs at intake and directs transport as needed.
• Introduce new patients to the treatment milieu and explain admission processes.
• Document intake information in the Tier program and maintain files.
