Job summary

Qualifications

• Master's degree in social work or equivalent required.
• Three years of relevant medical social work experience preferred.
• Knowledge of care management processes and ethics.

Responsibilities

• Provide social work intervention related to hospitalization and discharge planning.
• Collaborate with care teams to transition patients to appropriate care levels.
