UHS in Augusta, MI is seeking a Case Manager to provide quality substance abuse case management services, including bio-psycho-social assessments, intake tasks, and discharge planning. You will conduct group sessions and help develop group content, collaborating with nursing, admissions, and other staff to ensure smooth transitions into the treatment milieu.

Qualifications

• Undergraduate degree in social work, counseling, psychology, or mental health plus at least one year of substance abuse experience.
• Minimum of one year of supervised substance abuse experience.
• Ability to communicate verbally and in writing effectively.
• Knowledge of substance abuse treatment, group process, and discharge planning.
• Willingness to facilitate didactic and experiential groups.

Responsibilities

• Conduct bio-psycho-social assessments and intake tasks.
• Assist in discharge planning and development of group content.
• Lead and facilitate group sessions with adults with addictive and mental health disorders.
• Coordinate with Admission Center, Utilization Review, Nursing and Residential staff.
• Maintain intake paperwork and document in the TIER system.
• Ensure confidentiality and professional conduct in interactions with patients and staff.
