Job summary

CMJ RECOVERY AZ LLC is seeking a Case Manager to support clinical, non-clinical and medical staff in delivering comprehensive daily programming. You will conduct initial assessments, provide case management services, and assist with aftercare planning, discharge surveys, and documentation.

This in-person role requires HS diploma or GED, valid driver's license with clean record, and strong interpersonal and organizational skills.

Qualifications

• HS Diploma or GED required.
• Strong interpersonal and communication skills.
• Proven case management experience.
• Proficient with MS Office and EMR.

Responsibilities

• Assist clinical, non-clinical and medical staff in daily programming.
• Conduct initial assessments and provide case management services.
• Meet with clients weekly for case management.
• Complete documentation and regulatory reporting.
• Coordinate referrals to physicians, dentists and other providers.
• Facilitate aftercare planning and discharge surveys.
• Monitor urinalysis and breathalyzer screenings as indicated.
