Qualifications

  • Experience with co-occurring disorders (mental health and substance use).
  • Strong written and verbal communication skills.
  • Willingness to travel within the community and work flexible hours.

Responsibilities

  • Conduct intake assessments within 72 hours in settings convenient for the individual (home, hospital, community, etc.).
  • Determine service eligibility in accordance with Michigan Mental Health Code and Medicaid guidelines.
  • Provide crisis intervention and support as needed.
  • Develop preliminary person-centered treatment plans and crisis plans.
  • Educate individuals on services, recipient rights, complaint resolution, and health/safety procedures.
  • Initiate case management services and coordinate transitions to ongoing care.
  • Participate in triage and ensure all required documentation is completed accurately and on time.
  • Provide alternative recommendations and due process information when services are denied.
  • Maintain compliant, thorough, and timely clinical documentation.
  • Participate in on-call rotation, team meetings, supervision, and ongoing training.

  • Ability to work independently while collaborating within a multidisciplinary team
Additional Requirements
  • Valid Michigan driver’s license with a good driving record
  • Reliable personal vehicle and proof of insurance for work-related travel
