The role involves conducting bio-psycho-social assessments, assisting in discharge planning, leading group sessions, and developing group content. The position requires strong communication, organizational, and documentation skills, with experience in intake processes, motivational interviewing, and coordinating care across departments.

Qualifications

• Undergraduate degree in social work, counseling, psychology or mental health.
• Minimum of one year of substance abuse experience.
• Experience with intake, assessments, and discharge planning.
• Experience facilitating groups.
• Knowledge of Motivational Interviewing.
• Ability to document accurately and maintain confidentiality.

Responsibilities

• Assess prospective patients for treatment and determine admission eligibility.
• Assist in discharge planning and group session development.
• Document intake information into the system and maintain accurate records.
• Coordinate transportation and patient introductions to the therapeutic community.
• Collaborate with Admission Center, Utilization Review, Nursing and Residential staff.
• Communicate effectively with patients, families and referral sources.
