Job summary

Universal Hospital Services Inc. seeks a Case Manager to provide quality substance abuse case management, conduct bio-psycho-social assessments, assist in discharge planning, and help develop group content.

The role requires coordinating with Admission Center, Utilization Review, Nursing, and Residential staff to ensure smooth patient transfers and accurate documentation. The candidate should have an undergraduate degree in a related field and at least one year of substance abuse experience,

Qualifications

• Undergraduate degree in social work, counseling, psychology or mental health and a minimum of one year of substance abuse experience.
• Minimum of one (1) year of supervised substance abuse experience

Responsibilities

• Assess prospective patients for treatment, identifying psychosocial, substance use, mental health and medical issues for admission.
• Complete utilization review pre-certifications based on intake assessments.
• Administers breathalyzer and vital signs at intake and documents results.
• Arrange transportation as needed for patients.
• Introduce new patients to the therapeutic community.
• Explain evaluation and admission process to patients/referral sources.
