Job summary

Gaudenzia, Inc. is seeking a Case Manager to support clients with substance use treatment and related services. You will conduct intake interviews, manage a caseload, and coordinate with community resources to ensure comprehensive discharge plans.

The role requires a relevant clinical bachelor's degree and 2–3 years of case management or counseling experience, with opportunities for advancement to supervisory responsibilities.

Qualifications

• Bachelor's degree in a clinical discipline is required.
• Case Manager I requires 2 years of case management or counseling experience.
• Case Manager II requires 3 years of case management or counseling experience.
• Case Manager III requires 3 years with at least 1 year supervisory/administrative experience.

Responsibilities

• Treat all clients and staff with dignity and respect.
• Attend team meetings as required by Supervisor.
• Conduct intake interviews when needed and manage a caseload per program requirements.
• Complete progress notes within 24 hours of service delivery.
• Coordinate with resources to ensure treatment and discharge planning.
• Maintain relationships with service providers and referral sources.
