Job summary

South County Mental Health Center, Inc. is seeking a Substance Use Care Coordinator for our Mobile Response Team. The role coordinates care for individuals in crisis, tracks linkage to services, and follows up within 72 hours to ensure successful outcomes.

Qualified candidates hold a bachelor’s degree in a related field, two years’ experience with at-risk populations, and a Florida driver’s license. Full-time, on-site position with comprehensive benefits and documentation duties across multiple

Qualifications

• Bachelor’s degree in counseling, social work, psychology, or related human services field.
• Two years full-time or equivalent experience working with adults and children at risk.
• Must have a valid Florida Driver’s License.

Responsibilities

• Face-to-face assessment interviews with clients to determine substance use needs.
• Accompany clients to and link them with community resources as needed.
• Counsel clients and family members on relevant clinical issues when appropriate.
• Continuous outreach to all clients in caseload.
• Advocate for clients by locating treatment facilities or halfway houses.
• Complete applications for services and discuss client needs with providers.
• Coordinate provider care including psychiatrists, therapists, medication clinics, and external services such as hospitals, social service agencies, and housing agencies.
• Monitor treatment process to ensure required services are provided timely.
• Attend staff and community meetings.
• Documentation including progress notes, progressive reviews, and other reports on multiple platforms.
• Complete extensive monthly report to convey data to DCF.
• Conduct assessments as instructed as part of client’s medical records.
• Daily entries into Credible (EHR) schedule and COC module.
