Job summary

Mental Health Resource Center in Florida is seeking a dedicated Care Coordinator for the Suicide Prevention Team. This full-time role focuses on rapid follow-up, care coordination, and transition planning to support safety during high‑risk periods for individuals with serious mental illness or substance use disorders.

Qualifications

• Bachelor's degree in Social Work or related field from an accredited university is required.
• Minimum of two years of experience in human services or behavioral health is required; experience with adults with mental illness preferred.
• Proficiency in MHRC EHR System within three months of employment; proficiency in Microsoft Office and email are required.
• Strong communication and interpersonal skills; ability to work effectively with individuals, families, providers, and team members.

Responsibilities

• Complete initial and ongoing assessments including suicide risk and medical history.
• Conduct outreach to crisis stabilization units, inpatient facilities, EDs, and community partners.
• Develop individualized care and safety plans in collaboration with individuals and families.
• Provide safety planning and counseling on access to lethal means.
• Maintain proactive contact based on suicide risk including 24-hour follow-up.
