Job summary

Partners Health Management seeks a Mental Health Substance Use Care Manager to engage adults and/or youth in mental health/substance use services. This mobile role collaborates with hospitals, providers, and local stakeholders to plan, assess, refer, and monitor care across multiple sites in North Carolina.

You will coordinate education, referrals, and care management activities, linking members to benefits and services while conducting regular assessments and care plan updates in a

Qualifications

• Bachelor’s degree in a human service field with minimum two years of full-time experience, or four years if degree is in another field.
• Master’s degree in a human service field with one year of experience, or provisionally licensed.
• Current unrestricted professional licensure (LCSW-A/LCMHC-A/LCAS-A/LMFT-A) or RN with relevant experience.

Responsibilities

• Provide education, referrals, and care management activities across Physical Health, Behavioral Health, and related services.
• Link members to community health resources and coordinate access to benefits.
• Conduct Care Management Assessments and Care Plans.
• Lead Care Team meetings and monitor progress toward goals.
• Oversee care transitions between clinical settings and ensure proper documentation.
