Job summary

Endeavor Health is seeking an RN Patient Care Navigator for our Stroke Program at Edward Hospital in Naperville, IL. This per diem position supports acute stroke care coordination and works with neurologists, physicians, and APPs.

The role emphasizes responding to stroke alerts, assisting with acute interventions, and educating patients and families. Hours vary with clinic operations, including some weekends.

Qualifications

• Bachelor's degree in nursing and Illinois RN licensure.
• Minimum three years RN acute care experience in stroke care.
• Current CPR (BLS) certification.
• ACLS certification or obtain within 3 months of start date.
• NIHSS certification is required.

Responsibilities

• Coordinate stroke care across teams and respond to acute alerts.
• Assist with transport to emergency testing and transfer to higher level of care as indicated.
• Educate patients and families on stroke care.
