Job summary

Endeavor Health is seeking an RN Patient Care Navigator for the Stroke Program at Edward Hospital in Naperville, IL. This per diem role requires coordinating care across the stroke continuum, responding to acute alerts, and supporting interventions with a multidisciplinary team.

The ideal candidate has a Bachelor's in Nursing, IL RN licensure, BLS, ACLS (or within 3 months), and NIHSS certification, plus 3+ years of acute stroke experience. Expect flexible hours and weekend coverage as needed.

Qualifications

• Bachelor's Degree in Nursing from an approved program.
• Current IL RN licensure is required.
• CPR certification (BLS) from the American Heart Association is required.
• ACLS certification is required or to be obtained within 3 months of start date.
• NIH Stroke Scale (NIHSS) certification is required.

Responsibilities

• Coordinate care for stroke patients across the continuum with emphasis on acute stroke alerts and interventions.
• Assist with transport to emergency testing and transfer to higher level of care as indicated.
• Collaborate with the stroke coordinator and neurosciences team to meet comprehensive stroke center standards.
• Provide stroke education to patients, families, and healthcare staff.
• Serve as a clinical resource for stroke protocols, quality improvement initiatives, and regulatory compliance.
