Job summary

Endeavor Health is seeking an RN Patient Care Navigator – Stroke Program at Edward Hospital in Naperville, IL. Per diem role focused on acute stroke alerts, coordination of care, and collaborative practice with the stroke team.

The navigator will perform neurological assessments, support emergency interventions, and help maintain stroke program data while educating patients and staff. Requires BSN, 3+ years stroke RN, IL license, BLS/ACLS/NIHSS, weekends possible.

Qualifications

• Bachelor's Degree in Nursing (BSN) required.
• Minimum three years of RN acute care experience in stroke care.
• Current State of Illinois Registered Nurse licensure.
• Current CPR Certification (BLS) issued by AHA.
• Current ACLS Certification or obtain within 3 months of start date.
• Current NIHSS Certification.

Responsibilities

• Coordinate stroke patient care across the continuum with emphasis on acute stroke alerts and timely interventions.
• Collaborate with neurologists, physicians and the stroke team to ensure quality care.
• Provide stroke education to patients, families and healthcare staff.
• Assist with transport to testing and transfers to higher levels of care as indicated.
