Job summary

Endeavor Health is seeking an RN Patient Care Navigator for the Stroke Program at Edward Hospital in Naperville, IL. This per diem role supports stroke care coordination with clinicians and the multidisciplinary team, ensuring timely interventions and education for patients, families, and staff.

Qualified candidates hold aBSN, IL RN license, and BLS/ACLS/NIHSS certifications with 3+ years of RN acute stroke care experience. Weekend coverage may be required, with hours ranging 7:00 AM–7:30 PM.

Qualifications

• Bachelor's Degree in Nursing required.
• Minimum three years RN acute care experience, preferably in stroke.
• Current State of Illinois RN licensure and CPR (BLS).
• ACLS certification (or within 3 months of start).
• NIHSS certification and ongoing professional development.

Responsibilities

• Coordinate care for stroke patients with neurologists, physicians, and APPs.
• Support acute interventions and ensure timely evidence-based care.
• Maintain stroke program data and provide patient and staff education.
