Job summary
Endeavor Health is seeking a Nurse RN Patient Care Navigator for the Stroke Program at Edward Hospital in Naperville, IL. Per Diem and varied clinic hours support a dynamic team dedicated to acute stroke care coordination.
The role requires a Bachelor's in Nursing, IL RN licensure, and 3+ years of RN acute care experience in stroke. Certifications (BLS, ACLS, NIHSS) are expected, with ACLS able to be obtained within 3 months. On-site position with flexible weekend work as needed.
Qualifications
- RN with Illinois licensure and a Bachelor's degree in nursing.
- Minimum 3 years RN acute care experience, focused on stroke care.
- Current CPR (BLS) and ACLS certification (or within 3 months).
- NIHSS certification required.
Responsibilities
- Coordinate care for stroke patients with neurologists and APPs.
- Respond to acute stroke alerts and support timely interventions.
- Educate patients, families, and staff on stroke care.
- Ensure compliance with stroke program standards and data collection.
