Endeavor Health is seeking an RN Patient Care Navigator for the Stroke Program at Edward Hospital in Naperville, IL. This per diem position supports stroke patients from alert through transfer, with shifts that vary 7:00 AM–7:30 PM and weekend coverage as needed.

The role requires NCLEX-qualified RN with 3+ years in acute stroke care, IL licensure, BLS, ACLS (or within 3 months), and NIHSS certification. Per diem scheduling and site-based team collaboration are essential.

Qualifications

• Requires Bachelor of Science in Nursing (BSN) or equivalent.
• Minimum three years of RN acute care experience focusing on stroke.
• Current IL RN license and BLS/ACLS certifications.
• NIH stroke scale certification is required or obtained within 3 months.

Responsibilities

• Provide professional nursing care for stroke patients along the continuum of care.
• Assist with acute stroke alerts, interventions, and coordination with neurologists, APPs, and multidisciplinary teams.
• Educate patients and families; contribute to quality improvement and regulatory compliance.
