Job summary

Endeavor Health is seeking an RN Patient Care Navigator for the Stroke Program at Edward Hospital in Naperville, IL. This per diem role offers clinic hours and weekend coverage.

The navigator coordinates care with neurologists and the multidisciplinary team, handles acute stroke alerts, and supports timely, evidence-based interventions. The ideal candidate has a Bachelor’s Degree in Nursing, IL RN license, BLS, ACLS or eligibility, NIHSS certification, and 3–5 years in acute stroke or critical

Qualifications

• Bachelor's Degree in Nursing is required.
• Minimum three years of RN acute care experience in the treatment of stroke patients is required.
• Current State of Illinois Registered Nurse licensure is required.
• Current CPR Certification (BLS) issued by American Heart Association is required.
• Current ACLS Certification issued by American Heart Association, or obtain within 3 months of start date is required.
• Current NIHSS Certification is required.

Responsibilities

• Coordinate nursing care with neurologists, physicians, and APPs throughout the stroke continuum.
• Respond to acute stroke alerts and support timely, evidence-based interventions.
• Perform neurological assessments and communicate care plans clearly to the team.
• Assist with patient transport to emergency testing and transfer to higher level of care as indicated.
• Contribute to stroke program data, quality improvement initiatives, and regulatory compliance.
