Job summary

Advanced Center for Nursing and Rehabilitation in New Haven, CT seeks an experienced MDS Coordinator to manage the MDS/RAI process, care plans, and regulatory compliance in a skilled nursing setting.

The role requires an active RN license (LPN/LVN considered at some facilities), knowledge of MDS 3.0, RAI, CMS rules, and PDPM, and proficiency with EHR systems. Strong coordination, communication and analytical skills are essential.

Qualifications

• Active RN license or willingness to consider LPN/LVN in some facilities.
• RAC-CT certification preferred.
• 1–3 years of experience in long-term care, skilled nursing, or rehabilitation.
• Strong knowledge of MDS 3.0, RAI process, CMS regulations, and PDPM.
• Proficiency with electronic health records (EHR).
• Excellent organizational, analytical, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments according to regulatory timelines.
• Develop, coordinate, and update individualized care plans from resident assessments.
• Maintain CMS and state regulatory compliance and participate in surveys and audits.
• Support Medicare/Medicaid reimbursement through accurate coding and appropriate RUG/PDPM classification.
• Contribute to quality improvement initiatives and infection control activities.
