Job summary

Essential Healthcare Management seeks an experienced MDS Coordinator in Connecticut to lead the RAI process and ensure accurate MDS assessments in skilled nursing facilities.

You will develop care plans, coordinate with nursing staff, physicians, therapists, and other professionals while monitoring regulatory compliance and reimbursement under CMS and PDPM rules.

Qualifications

• Active RN license; some facilities may consider LPN/LVN candidates.
• RAC-CT certification preferred.
• Minimum 1–3 years of experience in long-term care, skilled nursing, or rehabilitation.
• Strong knowledge of MDS 3.0, RAI process, CMS regulations, and PDPM.
• Proficiency in electronic health records (EHR) systems.
• Excellent organizational, analytical, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments within regulatory timelines.
• Develop, coordinate, and update individualized care plans based on resident assessments.
• Ensure compliance with CMS guidelines and state regulations; participate in surveys, audits, and quality assurance activities.
• Support Medicare/Medicaid reimbursement through accurate coding and PDPM classification.
• Collaborate with billing, nursing, and therapy teams to monitor quality measures and drive improvements.
