Job summary

HealthCare Access Maryland (HCAM) provides person-centered care coordination for individuals transitioning from publicly funded substance use treatment to the next level of care. This State Care Coordinator role emphasizes collaboration, outreach, and diligent documentation to support recovery over up to 6 months post-transition.

The position requires coordinating intake, ISP development at multiple milestones, and regular client contacts, with on-site visits to treatment sites in Baltimore

Qualifications

• High School Diploma or GED; Bachelor's preferred.
• 2 years administrative support experience in a multidisciplinary team.
• Reliable transportation.
• Experience with substance use recovery population preferred.

Responsibilities

• Coordinate ongoing access to services per Service Plan.
• Attend follow-up meetings at facilities or HCAM offices.
• Submit mileage expenses monthly via Nexonia and report to Program Manager.
