Qualifications

• High School Diploma or GED with significant human services experience.
• Bachelor’s degree in Human Services preferred.
• 2 years of administrative support experience with a multidisciplinary team.
• Spanish language ability (speak, read, and write).
• Reliable transportation.
• Community Health Worker Certification preferred.

Responsibilities

• Educate clients on access to low-cost/no-cost health care services and benefits.
• Conduct home and telephone assessments to connect to primary, dental, and specialty care.
• Provide education about Maryland Medical Program benefits for eligible clients.
• Assist removing barriers to healthcare including housing, transportation, and more.
• Conduct home visits per the organization’s SOPs.
• Document and close cases within defined timelines and procedures.
