Job summary

About HealthCare Access Maryland (HCAM):

HealthCare Access Maryland (HCAM) is a nonprofit organization dedicated to making Maryland healthier by connecting residents to health insurance coverage and providing comprehensive care coordination. Job Summary:

The State Care Coordinator provides person-centered and recovery-oriented care coordination services that assist individuals who are transitioning from publicly funded and fee-for-service substance use treatment to the next level of care by connecting them with services that support their continuing recovery. This is accomplished through a compassionate and collaborative working alliance with individuals serving for up to 6 months post-transition from a substance use treatment facility.

Duties and Responsibilities
Administration
  • Coordinate and provide ongoing assistance accessing and engaging in services as outlined in the Service Plan.
  • Participate in follow-up meetings at the treatment facilities, residence of person served, HCAM offices, or other facilities as identified by individual or care coordination team.
  • The State Care Coordinators are responsible for submitting monthly Mileage Expenses through Nexonia in addition to sending the mileage reimbursement expense report to the Program Manager by 5th of the following month to ensure payment is approved in a timely manner
Case Management
  • The State Care Coordinator must schedule and complete the Client Intake within 5 business days of the referral received.
  • Clients that are enrolled in the care coordination program are required to complete an ISP (Individualized Service Plan) during intake, month 3, and month 6.
  • The State Care Coordinator is required to make 2 contacts with their client each month in-person or by telephone. The first contact is due before the 15th of the month, and the second contact by the 30th of the month.
  • All case management notes and required documents will be entered into eCW (client management database) within 24-48 hours of the completed Client Intake.
  • State Care Coordinators are required to complete a Client Satisfaction Survey for clients who are enrolled in the state care coordination program for 2 months, 4 months, and at discharge for Baltimore County.
  • State Care Coordinators will engage in a Client Audit each month with their immediate supervisor. The SCC will verify that all required documentation is uploaded into eCW, and all client notes are updated and completed.
Relationship Management
  • Build customer-oriented, collaborative relationships with treatment providers and clients.
Outreach/Field Activity:
  • The State Care Coordinators are responsible for visiting their assigned treatment sites twice a week for a minimum of 4 hours each day. During the visit, the coordinators are to engage with the clients by completing intakes, providing resources and tangible items to the clients.
Human Resources
  • Complete all required HCAM and funder required trainings.
Additional duties and responsibilities:
  • Other duties as assigned and based on needs of the program.
QUALIFICATIONS:
  • High School Diploma or GED with significant human services experience. Bachelor’s degree in a Human Services field is preferred.
  • 2 years of experience working in an administrative support capacity with a multidisciplinary team. Experience working within substance abuse recovery population preferred.
  • Reliable transportation required.
KNOWLEDGE, SKILLS & ABILITIES
  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365).
  • Organizational skills with the ability to meet a demanding workload
  • Detail oriented to complete requirements of files and contract compliance.
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • We practice the “Housing First” philosophy. Knowledge is preferred, support is required.
  • Excellent oral and written interpersonal and communication skills required
  • Proven ability to work efficiently and meet deadlines
Compensation

In compliance with Maryland's wage transparency requirements, the compensation for this role has been included in the posting.
