HealthCare Access Maryland (HCAM) seeks a Care Coordination Associate to support BCDSS permanency staff in coordinating health care services for children in out-of-home placement. Responsibilities include scheduling medical, dental, mental health, and specialty care, tracking services, and maintaining documentation.

The role requires a Bachelor’s degree in social work or health-related field, strong communication, and database proficiency for MATCH and CHESSIE systems.

Qualifications

• Bachelor’s degree in social work or health related field, or equivalent.
• Experience coordinating care/services for clients and strong interpersonal skills.
• Proficient with MS Word, Excel, Outlook and Gmail; familiar with databases.

Responsibilities

• Serve as a resource person for DSS permanency staff navigating health systems.
• Track health care services for children in out-of-home placement.
• Identify resources and make referrals to agencies.
• Document assessments, plans, and resolutions in database.
• Update MATCH and CHESSIE databases and answer calls.
