Job summary

Qualifications

Responsibilities

• Provide weekly clinical support to assigned clients.

• Track referrals for PRP, housing, substance use treatment, and primary care.
• Support client engagement, transitions, and continuity of care.
• Maintain accurate documentation in the EMR and ensure compliance.
