Job summary

Qualifications

• High school diploma or GED required.
• Lived experience in recovery from substance use disorder.
• Knowledge of the recovery process and recovery planning.
• Strong interpersonal and communication skills.
• Ability to use SBIRT and motivational interviewing techniques.
• Ability to build trusting, supportive relationships with patients and families.
• Skills in care coordination, community resource linkage, and follow-up outreach.
• Ability to document services and work collaboratively with the care team.

Responsibilities

• Support patients with substance use disorders throughout the care continuum.
• Serve as a role model, mentor, advocate, and motivator.
• Conduct brief interventions using the SBIRT model.
• Assist with recovery planning and treatment referrals.
• Provide follow-up support after discharge.
• Help connect patients to community support services, treatment, transportation, and appointments.
• Collaborate with nursing, social work, ED staff, and other members of the care team.
• Complete required documentation and reports.
• Participate in team huddles, meetings, and trainings.
