Job summary

HealthCare Access Maryland (HCAM) seeks a Licensed Social Work Care Coordinator to provide short-term care coordination for individuals and families using MD Medicaid. You will receive referrals, assess needs, partner with MCOs, providers, and community resources to remove barriers to care, and lead a team to apply best practices for psychosocial and behavioral health needs.

Qualifications

• BSW degree from an accredited program and Maryland licensure.
• Minimum of 2 years related experience in social work or care coordination.
• Clinical experience with pregnant women and/or infants preferred.

• Lead team meetings to discuss complex cases and share resources.
• Receive referrals, assess needs, and coordinate with MCOs and providers.
• Outreach to clients within 5–10 days of referral and address barriers to care.
• Document outreach activities and prepare narratives and reports.
• Collaborate with community agencies to connect clients to resources.
