Job summary

Westerville Post Acute is seeking a Staff Physical Therapist to evaluate residents, create individualized treatment plans, and document outcomes in medical records. You will supervise PT assistants and train staff while coordinating with physicians, nursing, residents and families to optimize rehabilitation goals.

Responsibilities include patient education on home exercise programs, safety techniques, fall prevention, and adherence to regulatory and payer requirements in a collaborative care

Qualifications

• Bachelor’s degree in Physical Therapy; Master/Doctorate preferred.
• Proficient in computer skills such as Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise PT assistants and training of staff as needed.
• Communicate progress and discharge planning with physicians, nursing staff, residents and families.
• Maintain timely, accurate documentation per facility and payer requirements.
• Educate residents and families on home exercise programs and safety techniques.
