Job summary

Buckeye Care and Rehabilitation seeks a Staff Physical Therapist to evaluate residents, develop treatment plans, and coordinate care with the health team. You will supervise PT assistants and educate staff, families, and residents on mobility and safety.

Strong computer skills and state licensure are required. Responsibilities include documentation, discharge planning, and participation in quality improvement initiatives within the facility.

Qualifications

• Bachelor's degree in Physical Therapy; license required.
• Masters/Doctorate in Physical Therapy preferred.
• Proficient in Net Health and Point Click Care software.
• Licensed Physical Therapist in the state with active license and CE requirements.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and function.
• Supervise PT assistants and educate staff on treatment programs.
• Maintain accurate documentation in medical records and ensure compliance with policies.
