Job summary

Barberton-Post-Acute is seeking an experienced Staff Physical Therapist to evaluate residents, develop and implement personalized treatment plans, and supervise PT assistants. You will coordinate with physicians, families, and interdisciplinary teams to support discharge planning and mobility goals.

The role emphasizes documentation accuracy, infection control, and ongoing education for residents and staff, with opportunities to contribute to quality improvement and rehab program development.

Qualifications

• Bachelor’s degree in Physical Therapy required; Master’s or Doctorate in Physical Therapy preferred.
• Licensed Physical Therapist in the state; ongoing CE requirements.
• Proficient with PT data entry in health information systems (Net Health, PCC).

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT assistants and aides; train staff as needed.
• Communicate progress and discharge plans with physicians, nursing staff, residents and families.
• Document timely and compliant records; participate in QA and facility rehab programs.
• Educate residents and families on goals, home exercise programs, and fall prevention.
• Recommend durable medical equipment to support independence.
