Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, coordinate with families and the health team, and maintain documentation in medical records.

You will supervise PT assistants, develop personalized treatment plans to improve mobility and independence, and participate in care conferences and quality initiatives. Current Ohio licensure is required, with strong communication and clinical skills emphasized.

Qualifications

• Licensed Physical Therapist with current state license.
• Bachelor’s degree in Physical Therapy; Masters/Doctorate preferred.
• Proficiency with Net Health and Point Click Care software.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise PT assistants and students in resident care.
• Communicate with physicians, families, and staff on progress and discharge planning.
• Maintain documentation per facility, state, and payer requirements.
• Educate residents and families on home exercise programs and safety.
