Job summary

Qualifications

• Bachelor's degree in Physical Therapy; Masters or Doctorate preferred.
• Licensed Physical Therapist in the state with active license in good standing.
• Proficient with Net Health and Point Click Care data entry.

Responsibilities

• Evaluate residents to determine PT needs, functional limitations, and rehab potential per physician orders.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
• Supervise PT Assistants in resident care and related activities per state practice act.
• Communicate progress with physicians, nursing staff, residents, and families; participate in discharge planning.
• Complete timely, accurate documentation in accordance with facility, governmental, and third-party payer requirements.
• Maintain a safe treatment environment through infection control and equipment maintenance.
• Educate residents, families, and caregivers on goals, home exercise programs, safety techniques, and fall prevention.
• Assist with ordering of durable medical equipment to support independence.
