Job summary

Centerville Post Acute is actively seeking a licensed Physical Therapist to join our rehabilitation team in Centerville, OH. The PT will evaluate residents, create individualized plans, and deliver skilled therapy to restore mobility and independence in a skilled nursing and post-acute setting.

The role requires OH licensure, CPR, and a patient-centered approach. Collaboration with physicians, nurses, and therapists is essential, along with thorough documentation and commitment to quality care.

Qualifications

• Licensed Physical Therapist in Ohio (OT-PT license as required).
• CPR certification required.
• Minimum of 2 years PT experience preferred, post-acute care experience desired.

Responsibilities

• Evaluate residents and develop individualized PT treatment plans.
• Provide skilled PT to improve mobility, strength, balance and function.
• Educate residents, families, and caregivers on exercises, safety, and discharge planning.
• Complete accurate clinical documentation per facility and payer requirements.
• Participate in interdisciplinary care planning meetings and collaborate with the care team.
• Recommend adaptive equipment and environmental modifications as appropriate.
• Ensure compliance with Medicare/Medicaid documentation guidelines and regulatory standards.
