Job summary

North Royalton Post Acute is seeking a licensed Physical Therapist to evaluate residents, develop individualized treatment plans, and improve mobility and independence. You will work with physicians, families, and an interdisciplinary team, supervise PT assistants, and maintain compliant documentation.

Responsibilities include training staff, participating in care conferences, coordinating discharge planning, and promoting safety and fall prevention through education and home exercise programs.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• State licensure in good standing.
• CPR certification, if required by licensure.
• Continuing education to maintain licensure.
• Experience supervising PT assistants preferred.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and oversee resident care.
• Communicate progress and discharge planning with physicians, families, and staff.
• Complete timely documentation in accordance with regulations.
• Provide in-services and training for staff as needed.
• Facilitate ordering of necessary DME for independence and safety.
• Participate in Resident Care conferences and Rehabilitation meetings.
