Job summary

Bridge Home Health & Hospice is seeking a Speech-Language Pathologist to implement standards of care for speech-language pathology services in a home health setting. The role involves evaluating patients, planning care, administering therapy, and coordinating with physicians to ensure comprehensive care.

The position requires a master’s or doctoral degree in SLP, state licensure (or eligibility), CPR certification, and a CA driver’s license.

Qualifications

• Must hold a master’s or doctoral degree in speech-language pathology (SLP).
• Must be licensed as a speech-language pathologist by the State or be eligible for licensure.
• Minimum of one year of clinical SLP experience.
• Current CPR certification.
• Must possess a valid California driver’s license with insured vehicle.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Conduct evaluations and assess home environment as appropriate.
• Identify functional speech defects and develop a care plan.
• Administer speech therapy using specialized techniques.
• Recommend communication devices as needed.
• Coordinate with referring physician and care team.
• Participate in case conferences and staff meetings.
• Prepare and submit notes per policy.
• Maintain clinical competency in SLP practice.
• Refer patients to other services when appropriate.
• Instruct patient and caregiver in treatment regimen.
