Job summary

Bridge Home Health & Hospice is seeking a Speech-Language Pathologist to implement standards of care for home-based speech-language pathology services. The role requires mastery of SLP evaluation, treatment planning, and collaboration with physicians to support comprehensive care.

Responsibilities include conducting home assessments, administering therapy with specialized techniques, and educating patients and caregivers.

Qualifications

• Master’s or doctoral degree in speech-language pathology (SLP).
• Licensed as a speech-language pathologist in the state/province of practice.
• 350 clock hours of supervised clinical practicum (or in progress).
• 9+ months of supervised full-time SLP services after degree.
• National examination in SLP successfully completed.
• Minimum 1 year of clinical SLP experience.
• Demonstrates good verbal and written communication and organization skills.
• CPR certification current.
• Valid Driver’s License with insured vehicle.

Responsibilities

• Conduct evaluations and assess home environment for speech-language needs.
• Identify functional speech defects and plan care to improve function.
• Administer speech therapy program using specialized techniques/equipment.
• Recommend communication devices/aids as indicated.
• Coordinate with referring physician and care team for total care.
• Participate in case conferences and other required meetings.
• Prepare and submit notes per organization policy.
